Service Coordination is the active assistance offered to persons with developmental disabilities as they navigate the community and various service systems in pursuit of the necessary
and desired services and supports that will assist them in achieving and/or maintaining their personal goals.
Today, people with disabilities are seeking control over their lives. Their focus on choice and
self- determination is directed towards all areas of life, but especially those areas that affect home, work, friends, finance, health and leisure. People with developmental disabilities have the right to determine
their personal goals and decide which supports and services they want and need.
The resources, supports and services for people with developmental disabilities continue to grow in number and complexity.
It is a challenge for people with developmental disabilities or their families to keep up with the changes and to maximize the opportunities. Many people turn to professionals for assistance. Service Coordinators help people explore what they want and need in life and then assist them in getting it. They work in partnership with the person and/or family to develop, implement and maintain the person's life plan. Service Coordinators assist people to attain the highest quality of life and live as independently and productively as possible.
